2012 Player Registration - Printable registration form for Spring 2012.

Medical Release - Release form, filled in by parents and carried by the team manager

Volunteer Application - This must be completed by all volunteers who will have regular and ongoing contact with player participants in MSLL Little League prior to any games, practices, team meetings. In other words, all managers, coaches, board members. A copy of your driver's license must also be submitted.

Little League Insurance - What parents should know about Little League insurance.

Accident Claim Form - In case of injury at a practice or game, this form must be completed and sent to Peter Enzien, MSLL's Little League President.
